“It is a reperfusion injury!” I shouted back. By removing the tight cast and releasing the physical pressure, all the trapped toxins and bad blood from his dying arm were rushing straight back into his main bloodstream, poisoning his heart. “Hook up a full bag of saline solution immediately! Give him two amps of bicarbonate and prepare the medical tray for a central line! We also need maintenance down here with bolt cutters to remove this chain!”

I worked as fast as my hands could move, relying entirely on my medical training while my mind raced. As I prepped the side of his neck for the central line, my fingers touched his sharp collarbone. The boy was impossibly thin, allowing me to count every single rib pressing against his fragile skin. He had clearly been starved for years behind closed doors.

“Why would she keep his baby teeth inside the cast?” Brandon asked in a shaky voice as he spiked the IV bag. “Why would anyone do something so insane?”

“Because she could never take a stolen child to a real dentist,” I answered through gritted teeth while inserting the wire into his neck. “When a child loses a tooth at home, you have to hide it so no one finds out he exists. She bound his physical identity to his own body so he would become his own portable prison.”

“They were his childhood memories,” Victoria’s cold voice called out from the wall where the guard held her down. “He was growing up far too fast, Doctor. The outside world is filthy, so I kept him completely pure and small. He did not need anything from the outside world because he only needed me.”

A wave of intense nausea washed over me as I finished securing the line. I swore to myself that I would never allow a monster to walk out of my hospital again.

“Get her out of my sight right now,” I said to Sam, looking him dead in the eye. “Take her down to the holding cell and do not stop watching her until the police put real handcuffs on her wrists.”

Sam nodded firmly, pulling Victoria up by her arms and marching her straight out of the trauma bay. She did not fight him physically, keeping her dark eyes locked on the boy until the sliding glass doors closed between them.

Ten minutes later, the double doors of the ER bay burst open, and Detective Thomas Bradley walked into the hallway. He was a man in his mid-fifties with a tired face, wearing a heavy coat and carrying the quiet exhaustion of someone who had spent decades dealing with terrible crimes. He took a single step into Trauma 2, smelled the air, and stopped in his tracks.

“Good Lord,” Thomas muttered, pulling a cloth from his coat to cover his mouth. He looked at the plastic bag of teeth on the metal tray, then at the rusted iron chain, and finally at the small boy lying on the bed. “Please tell me this is not the missing child from Ohio, Rachel.”

“It is Mason Albright, Thomas,” I said softly as a hospital worker carrying yellow bolt cutters approached the bed. “We found his old primary school identification card hidden inside the cast structure.”

Thomas closed his eyes for a long moment while his jaw tightened in anger. Everyone in law enforcement knew Mason’s face, and finding him in this terrible condition was heartbreaking.

“Cut the lock off right now,” I told the maintenance worker.

With a heavy metallic snap that sounded like a gunshot, the steel lock broke apart. Hannah and I used medical tools to carefully lift the rusted links off his raw, weeping skin. The iron had fused with his wounds, causing fresh blood to pool on the white bedsheets as each link came away.

As the heavy chain clattered into a metal basin, a small sound came from the boy’s throat. Everyone stopped moving immediately.

Mason’s cracked lips were moving slightly as his heavy eyelids fluttered open. His dark eyes were cloudy, unfocused, and filled with deep fear. He did not look at me, the detective, or the nurses standing nearby. He only looked down at his bare arm where the heavy cast used to be.

His breathing became rapid as he tried to pull his arm away, but he was far too weak to move his body.

“Mason,” I whispered gently, leaning over the bed so he could see my face. “You are completely safe now, sweetie. I am a doctor, and you are inside a hospital.”

He did not seem to hear my words at all. He kept staring at his exposed wrist while a single tear ran down his dirty cheek.

Then, in a voice so faint it sounded like dry paper dragging across concrete, he spoke words that chilled me to the bone. “Please put it back on me,” the eight-year-old begged, his body trembling as he tried to hide his face. “Please put it back right now. If she sees that I am growing bigger, she will put me back inside the box.”

For three long seconds, no one in the room dared to move or speak. I looked down at Mason as he curled his small body into a ball, trying desperately to hide under the thin blanket. He did not see us as medical professionals who had just saved his life. He saw us as people who had just exposed him to terrible punishment.

Detective Thomas Bradley was the first person to break the heavy silence. “What box is he talking about?” Thomas asked in a raspy voice, stepping closer to the bed. “Rachel, tell me he is just confused from the high fever.”

Before I could answer his question, the heart monitor erupted into a continuous, high-pitched alarm sound. The screen showed a chaotic, fatal rhythm as his weak heart began to fail completely. His body had taken too much physical trauma from the infection, the starvation, and the sudden rush of bad blood. His eyes rolled back into his head while his chest shuddered violently.

“He is in cardiac arrest!” I shouted, jumping onto the side of the gurney to begin chest compressions immediately. “Brandon, bring the crash cart over right now! Hannah, give him one milligram of epinephrine through the IV line! We need a breathing tube in his airway immediately!”

I locked my elbows and began pressing down on his tiny chest with all my strength. Performing CPR on a severely starved child is a horrifying experience because there is almost no natural cushion. His small chest cavity felt brittle, and on my third compression, I felt the sharp pop of his thin ribs breaking beneath my hands.

Hot tears spilled over my eyelashes, but I refused to stop moving. “Stay with me, Mason!” I yelled while pumping his heart for him. “Do not give up right now! Do not let that awful woman win!”

“The medication is in!” Hannah reported, her face completely pale as she adjusted the lines.

Brandon wheeled the red cart to the head of the bed and handed me the electric paddles. “The pads are placed, Dr. Vance! The machine is charged to fifty joules!”

“Everyone clear the bed!” I called out, pressing the buttons firmly.

Mason’s small body jerked upward off the mattress before falling limp against the wet sheets. I stared up at the monitor, but a flat line continued to scroll across the digital screen.

“Charge it to one hundred joules right now!” I ordered, my hands shaking. “Clear!”

I shocked his chest a second time, sending a loud pulse through his body. The smell of singed skin mixed with the foul odor of his arm.

Beep…

A long pause followed the sound.

Beep… Beep…

A slow, struggling heart rhythm finally appeared on the monitor screen. It was weak, but it was steadily holding.

“We have a rhythm back,” I gasped, stepping down from the bed with my scrubs soaked in sweat. “His pressure is still very low, but he is breathing.”

The specialized airway team rushed into the bay, inserting a plastic breathing tube down his throat to connect him to a ventilator machine. The steady rhythm of the mechanical breath taking over was a massive relief. I walked over to the sink to scrub my hands with hot water and soap, trying to wash away the physical memory of his breaking ribs.

Thomas stepped up to the sink beside me, looking at his own face in the mirror. “Is he stable enough for me to leave his side?” Thomas asked quietly.

“He is in a stable coma now,” I explained while drying my hands with a paper towel. “The infection is inside his blood, so we are giving him strong antibiotics. It will be a very slow recovery.”

Thomas nodded slowly before unhooking the radio from his belt. “Dispatch, this is Detective Bradley. Send a full SWAT team, crime scene officers, and child services to 1420 Glenwood Drive, the Sterling residence. Tell them to bring heavy tools to break through walls because we are searching for a hidden room.”

“Copy that, Detective,” the radio operator replied.

Just as Thomas put his radio away, the sliding glass doors opened once again, and a tall man walked briskly into the emergency department. He wore a high-end tailored suit, had grey hair along his temples, and carried an attitude of absolute authority. He marched past the front desk, ignoring the nurses who tried to stop him.

“Where is my wife right now?” the man demanded, his loud voice echoing through the corridor. “I am Douglas Sterling, senior partner at Sterling & Vance Law, and I want to know why my wife called me saying she was attacked by hospital employees!”

I felt the blood drain from my face as I realized who he was. This was the husband who lived in the very same house while a starved child was chained in the dark beneath his feet.

Thomas moved forward instantly, blocking Douglas from entering the trauma room. “Mr. Sterling,” Thomas said, holding up his gold police badge. “I am Detective Thomas Bradley. Your wife is currently in police custody down the hall.”

Douglas scoffed loudly, adjusting his silk tie with an annoyed expression. “In custody for what? Being a protective parent? My wife told me a doctor here became aggressive and threatened her over a simple medical cast. I assure you that my legal team will file a massive lawsuit against this facility tomorrow morning.”
